HAS FAMILY FOUNDATION Funding request preparation checklist This is a preparation aid, not an application or a submission channel. First gifts are planned for 2027. Consult the website for confirmed application dates and submission details. 1. ORGANIZATION - Full legal name and operating name - Qualified-donee type and registration number or other evidence of status - Mailing address, website and primary contact information OUR PREFERENCE: MATCHING COMMUNITY GIVING We encourage requests for HAS to match gifts raised through local fundraising activities. Matching is a preference, not a requirement for every request. Any match amount and terms require Board approval and available funding. 2. REQUEST - Amount requested and total project/program budget - General support or a designated purpose - Brief description of intended use (up to 250 words) - Proposed location, start date and completion date 3. PUBLIC BENEFIT - Community need or opportunity - People and communities expected to benefit - How participants or beneficiaries are selected, where relevant - Public accessibility, where relevant - Activities, expected results and how success will be assessed - Sustainability or next steps 4. BUDGET AND SUPPORTING MATERIAL - Project/program budget, with the requested contribution identified - Other committed and pending funding - For matching requests: local fundraising activity, goal and timeline - Community contributions already raised or pledged (identify each separately) - Matching amount requested from HAS - Recent annual report/public information, if available - Financial statements or a budget summary, if available - Relevant plans, quotes or letters of support 5. RESPONSIBILITY - Disclose relationships with Foundation directors, committee members, employees, donors, members or related persons - Identify who will confirm receipt and report on the gift if requested - Ensure the information can be certified by an authorized representative The Foundation currently makes gifts only to qualified donees. Gifts are paid directly to the approved legal entity. There is no application fee. All gifts require Board approval; submission does not create an entitlement to funding.
